Person-Centered Plan Facilitator Lead with Oparc
Location: Montclair, CA
Timing: Normal days and hours are Monday through Friday, 8:00 a.m. – 4:30 p.m. The position is a non-exempt position. Hours and days may very based on the needs of the position.
Salary: $20.19-$23.00/ hour
Function: The Person Centered Plan Facilitator Lead is responsible for overseeing, developing and facilitating the development of Person Centered Plans for OPARC Inland Regional Center individuals and for leading the Changemaker team of Progress4Providers.
Responsibilities/Duties:
- Develop Person Centered Plans for OPARC Inland Regional Center individuals.
- Review and provide guidance on PCPs developed by other staff members.
- Track all PCPs developed and reviewed.
- Schedule and hold IDT meetings in order to develop PCPs, with the individual’s circle of support.
- Work closely with the Integration Facilitator to convey desires/needs revealed in the development of PCPs.
- Work in collaboration with the PCP Facilitator and Case Managers and conduct monthly trainings with core team members.
- Communicate often with the individual’s program staff members and Case Manager(s).
- Meet regularly with supervisor to report on progress, issues, etc.
- Develop good relationships with people supported, CSCs, family members, care providers in order to facilitate meaningful PCPs.
- Actively participate in Progress4Providers Meetings and Changemaker team to identify priorities for change, sprint planning and implementation.
- Support and refine, if appropriate, procedures for the implementation of Person Centered Thinking applications producing the Person Centered Plans.
- Supervise and evaluate staff and their plans’ effectiveness.
- Assign and ensure the development of PCPs is occurring evenly across the agency.
- Meet grant benchmarks as required by contract and produce reports of progress reflecting the progress of plans and sprints for Progress4Providers.
- Attend Community of Practice and staff meetings.
Other Responsibilities/Duties:
•Ability to travel to OPARC sites, meeting locations, IRC, etc.
•Ability to work remotely or at various sites
•Professionalism – the PCPs must be a role model for all other OPARC employees – in regard to dress, language, conduct, and non-discrimination
•Organized, detail-oriented and able to meet deadlines
•Excellent computer skills
•Excellent communication skills, verbally and in writing
•Ability to work independently and as part of a team
•Ability to learn Person Centered Thinking/Planning philosophy and documentation
•Basic competency in: Microsoft Office Suite, including Word, Excel, and Outlook; behavior modification techniques; grammar; spelling; filing; record keeping
•Ability to maintain confidentiality
Education/Experience:
One year of experience with OPARC, to understand our program services and individuals we support. Prefer: Case Management experience. BS or BA degree in a related field.
Other Requirements:
Must have a clear driving record, valid CA driver’s license, 5 years driving experience, safe reliable transportation and proof of vehicle insurance. Able to lift up to 25 lb by self, standing, walking, sitting, pushing, bending, repetitive motion and pulling may be required. Proper handling/use of chemicals for cleaning and use according to manufacturer’s labeled instructions. Current CPR and First Aid Certification.
